Assessment Tool for Temperature Exposures of Lilly products

1. The Tool is designed for global use. Some of the listed products have different trade names, or are not available, in certain countries.
2. The Tool may only be used by health care providers and licensed wholesalers/distributors with the transaction history of each batch.
3. The Tool may only be used to assess accidental temperature exposures of Lilly parenteral products AFTER the exposure occurred.
4. The Lilly products being assessed must be unexpired and in the original Lilly packaging.
5. The temperature and total time of the exposure must be known.
6. The Tool assesses one temperature exposure at a time; contact Lilly for assessment of multiple exposures of the same product.
7. Products being assessed must be returned to the correct storage conditions stated on the product packaging before assessment.
8. The result only applies the assessed exposure and may not replace the required storage conditions or predict the stability impact of future exposures
9. Each new exposure must be re-evaluated.
10. The Tool cannot be used to assess exposure of the medicine to light.
